FINANCIAL
We are currently in network with the following insurance companies: United Healthcare, Blue Cross Blue Shield, Golden Rule,
Signature Wellness Plan, Great-West Healthcare, Principal Life Insurance Co., Health Allies, Access One Consumer Health,
Medical Resource, Secure Horizons, Pacificare Health Systems, Evercare, Premera Blue Cross, MedRisk, Cypress Care,
Universal SmartComp, TechHealth, Rockport, Health Systems International, Integrated Health Plan, and Align Networks.
As of May 1, 2012, we will no longer submit claims to insurance companies. We will however, provide you with the documentation required to submit for reimbursment.
CANCELLATIONS
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice, so that we can offer this appointment to another patient. A fee of $20.00 may be charged if a patient does not honor this policy.
